Linking forms to treatments and categories

Attaching a form to a treatment is how you scope it, so a patient booking lip filler gets the lip filler consent and nothing irrelevant.

Per treatment

  1. Go to Settings > Treatments.
  2. Open the treatment you want to configure.
  3. Under Forms for this treatment, choose the forms it needs and save.

Per category

If every treatment in a category needs the same paperwork, set it once at the category level instead. Select the category and use Forms for this whole category. A form attached to a category applies to every treatment in it, so you do not need to add it against each treatment individually, and it keeps covering new treatments you add to that category later.

Attaching is the whole instruction

There is no second setting to arm. Once a form is attached to a treatment or a category it goes out with bookings for those treatments and no others: consent, medical history and pre-information before the appointment, aftercare after the visit.

That is also why attaching a form and sending it to every patient are alternatives rather than additions. A form attached to nothing can be switched to Send to every patient, on every booking on its own page. While it is attached to a treatment that switch is not available to turn on, because the attachment has already said who gets it. To send an attached form to everybody, remove its attachments first.

You attach forms here, and the switch lives on the form's own page, so each screen now shows you both halves. The form's page names everything it is attached to in the Attached to panel beside the switch, and the top of the Forms page names any form whose setting and attachments do not agree, says what is actually happening to it, and gives you the two ways to settle it.

How it all combines

When an appointment is booked, the patient is asked for:

  • the forms attached to the booked treatment,
  • plus the forms attached to that treatment's category,
  • plus any form attached to nothing that is set to go to every patient.

Duplicates are removed, a form the patient completed inside its own re-ask window is skipped, and only forms with a published version are ever sent. So you can be generous with attachments without patients being asked twice.

One thing category attaching cannot do: reach a treatment that is not in a category. If you have treatments sitting outside your categories, attach their forms on the treatment itself.
