Editing a form
Each form is built from questions you control through the question builder.
The question builder
The builder lets you add, reorder and configure questions. Typical question types cover free text, yes or no, multiple choice and signature capture. Mark questions as required so a patient cannot submit the form without answering the ones that matter.
Editing safely
Because published versions are immutable, editing works like this:
- Open the form and make your changes in the builder. You are working on a draft, not the live version.
- Review the questions and required fields.
- Publish a new version when you are happy.
From that point, new sends use the new version, while every form already signed keeps the version the patient saw. Nothing already agreed is touched.
"Ask for patient details first"
This switch adds a details section to the top of the form where the patient confirms their phone number, date of birth, address, GP and next of kin. Anything they correct is written back to their patient record automatically, so it doubles as a check that your records are current. It is usually worth keeping on for your medical history questionnaire; a blank answer never overwrites what you already hold.
Practical tips
- Keep questions clear and plain so patients answer accurately.
- Only mark a question required if a missing answer should genuinely block submission.
- Publish once you are confident; a stray draft will not affect patients until published.
