Patient and practitioner signature requirements

Each form declares whose signatures it needs, and the app enforces the patient side at submission.

Setting the requirements

In the form editor, under Settings, there are two checkboxes:

• Requires patient signature. The patient must draw their signature in the signature box and type their full name before the form will submit. If either is missing, the form lists exactly what is outstanding and scrolls them to it.
• Requires practitioner signature. Records that this form should also carry a practitioner's signature, for use on consent documents your protocols say must be counter-signed.

Seeing requirements at a glance

The Forms page has a Signatures column showing a tick or dash against Patient and Practitioner for every form, so you can audit your whole library in one look.

What the patient sees

When a patient signature is required, the form ends with a Your signature section: a drawing box, a typed full name field, and the confirmation wording that by signing they confirm their answers are accurate to the best of their knowledge. The signature is stored with the submission against the exact published version they signed.

Which forms should require signatures?

Consent and medical history almost always should. Pre-information and aftercare forms are usually informational, so they are often sent without a signature requirement.
