Signing off a treatment record

Signing off marks a treatment record as complete. It does two things at once: it locks the map so it can no longer be edited, and it deducts the products used from stock.

How to sign off

  1. Open the record's treatment map and make sure everything is entered correctly: points, products, quantities and batch numbers.
  2. Press Save map first. The Complete & sign off button is disabled while you have unsaved changes.
  3. Press Complete & sign off and confirm.

What happens to stock

Each product line on the map is matched against your Stock list. The confirmation message tells you exactly what happened, for example how many product lines were deducted. If a product name is not in your stock catalogue, the record still saves in full but that line is listed as not deducted, so you can adjust stock by hand if needed.

After sign-off

The map becomes read-only and shows the date it was locked. You can still view and print it, and the record remains on the patient's history. What was documented on the day stays exactly as it was written, which is the point of signing off.

Adding to a signed-off record

Locked does not mean finished. If something needs adding later, a phone call the next morning or a detail you did not have at the time, use Add amendment on the record. The amendment appears underneath the original as its own dated, attributed entry, and the original stays exactly as it was written.

That is deliberate and it is how clinical records are meant to work: the record answers "what was known, and when", so it has to read in order and show its own amendments rather than quietly change. A record you can rewrite is worth much less if you are ever asked to produce it.
