A good medical history form screens for the things that change your treatment plan: conditions, medication, allergies and previous treatments.

Building the questions

Yes/No questions are the backbone of medical screening, and the builder has a feature made for them: tick Ask for details when answered YES on a Yes/No question and the patient is shown a Please provide details box the moment they answer yes. "Are you taking any medication?" followed by an automatic details box gives you the specifics without a wall of optional text fields.

Mark the questions that matter as Required so the form cannot be submitted with them blank.

Collecting patient details

Tick Ask for patient details first in the editor and the form opens with a details section: date of birth, phone, address, gender, GP name and address, and next of kin. Anything already on the patient's record is pre-filled, so returning patients only fix what has changed.

Not asked again for a year

A medical history questionnaire is normally attached to no treatment and set to Send to every patient, on every booking, so it goes out automatically whenever an appointment is booked, whatever the patient booked.

A patient who completed it inside its re-send window is not asked for the whole thing again. They are sent the short Confirmation of No Changes form instead, which asks them to confirm that nothing has changed, so their record is refreshed without the full questionnaire every visit. Twelve months is the usual window. Once it passes, the full questionnaire comes back and the cycle starts again, and nobody is ever sent both on the same booking.

One caution: the short form is recognised by its name, so if you rename your "Confirmation of No Changes" form the editor asks you to confirm, because returning patients would stop receiving it.

You can always send it manually in between from the patient's record if something has changed.
