Logging a complication or adverse event
When something goes wrong after a treatment, log it on the patient's record. A clear, dated complication log is your best protection medico-legally, and it makes sure follow-up actually happens.
Logging it
- Open the patient and find the Complications section.
- Press Log complication.
- Fill in the form:
- Date it occurred and severity: mild, moderate, severe or life-threatening.
- Type, with suggestions such as bruising, swelling, infection, vascular occlusion, allergic reaction or nodule.
- Link to treatment record, so the complication is tied to the visit that caused it.
- What happened (required) and the action taken.
- Implicated product and batch no. where relevant.
- A follow-up plan and a review by date.
Staying visible until resolved
While a complication is open, an amber banner appears in the left rail of the patient record showing the number of unresolved complications; clicking it jumps to the log. It stays there until you deal with it.
Resolving
When the issue has settled, type an outcome (optional) and press Resolve on the entry. Resolved complications stay on the record with their resolution date, and you can Reopen one if it flares up again. Entries are never deleted; the history is the point.
