Bringing in your own form wording

Many clinics already have approved forms, often from their insurer or training provider. You do not have to rebuild them by hand: paste the wording in and AesthetiClinic turns it into an e-signable, versioned form.

How to import

  1. Go to the Forms page and click Add your own.
  2. Give the form a name, for example Lip filler consent (our wording), and choose its type.
  3. Paste the full text of your existing form into the Paste your form's wording box.
  4. Click Create from my wording.

Refine it afterwards

The imported form becomes a normal clinic form, so you can open it with Customise and tidy the layout in the editor: split content into information blocks, add Yes/No screening questions, and set which signatures it needs. When you are happy, publish it and it is ready to send.

Whose responsibility is the content?

Your content, your responsibility. The library forms in AesthetiClinic are starting templates to review and adapt; they are not legal advice. If your insurer has approved a specific wording, importing it with Add your own is the safest route, because patients then sign exactly the words your cover expects.
