Bringing in your own form wording

Many clinics already have approved forms, often from their insurer or training provider. You do not have to rebuild them by hand: paste the wording in and AesthetiClinic turns it into an e-signable, versioned form.

You need to be the clinic owner or have access to the clinical record to import forms.

How to import

1. Go to the Forms page and click Import my forms.
2. Give the form a name, for example Lip filler consent (our wording), and choose its type.
3. Paste the full text of your existing form into the Paste your form's wording box.
4. Click Create from my wording.

If you want separate answer fields from a written list of questions, use Create form and Paste questions (optional) instead. Check the parsed preview, then press Create these questions.

Refine it afterwards

The import opens straight in the editor as a normal clinic form with version 1 already published. Your pasted wording is kept in one information block. Consent and medical history imports also get a required acknowledgement question and require a patient signature.

Tidy the layout in the editor if needed: split content into information blocks, add Yes/No screening questions, and set which signatures it needs. Pasted questions do not become answer fields automatically. Press Publish new version to apply any wording or question changes before you send it.

Whose responsibility is the content?

Your content, your responsibility. The library forms in AesthetiClinic are starting templates to review and adapt; they are not legal advice. If your insurer has approved a specific wording, importing it with Import my forms is the safest route, because patients then sign exactly the words your cover expects.
