How form templates and versions work

Forms in AesthetiClinic are built around templates and versions, which keeps your signed records trustworthy.

Templates from the library

You start by cloning a template from the form library, then adapting it to your clinic. Cloning means you get a sensible starting point rather than a blank page, while still owning and editing your own copy.

Versions are immutable

When you publish a form, that becomes a fixed version. Published versions are immutable: they never change. If you need to alter the wording, you publish a new version rather than editing the old one in place.

Signed forms keep their version

This is the important part. When a patient signs a form, the signature is tied to the exact version they saw. If you later publish a new version, every previously signed form keeps the wording the patient actually agreed to.

That means you can always prove what a patient consented to, word for word, on the day they signed. Editing live forms would quietly rewrite history; versioning makes that impossible.
