Creating a form from scratch

If none of the library templates suit, you can build a form of your own from an empty page.

Creating the form

  1. Go to the Forms page.
  2. Click Create form.
  3. Give it a name, for example Anti-wrinkle consent, and choose its type: Consent, Medical history, Pre-information or Aftercare. The type cannot be changed after creation, so pick carefully.
  4. Click Create & edit. You land straight in the editor with an empty form.

Adding questions

In the editor, use the buttons under the question list to add a Yes/No question, Text answer, Multiple choice or Information block. Each question can be reordered with the up and down arrows, marked as Required, and deleted with the bin icon. A Yes/No question can also ask for details when answered YES, which is ideal for medical screening questions.

Tick Ask for patient details first if you want the form to open with a details section (name, date of birth, contact, GP and next of kin) before your questions.

Publishing

Your changes are a draft until you click Publish new version. Only published forms can be sent to patients, and the editor shows an amber Unsaved changes note whenever there is something still to publish.

Your new form appears on the Forms page under the My clinic filter, marked with a Custom badge.
